鞋跟高度对青年女性下楼梯髌股关节力学特征的影响

The Influence of Heel Height on the Mechanical Characteristics of Patellofemoral Joint during Stair Descending in Young Female

  • 摘要: 为研究下楼梯过程中鞋跟高度对青年女性髌股关节力学特征的影响,采用Vicon红外运动捕捉系统和三维测力台同步采集25名青年女性分别穿着1、3、5和7 cm四种高跟鞋下楼梯的膝关节运动学和动力学数据。采用数字公式,计算青年女性髌股关节应力和反作用力等指标。使用单因素重复测量方差分析对指标进行统计分析。结果显示,与平跟相比,高跟、中跟和低跟状态下的髌股关节应力、髌股关节反作用力、股四头肌肌力、膝关节伸展力矩第二峰值显著增加。因此,女性下楼梯时,穿着3 cm以下的高跟鞋,降低髌股关节反作用力和应力,有助于降低髌股关节痛的发生概率。

     

    Abstract: To investigate the influence of heel height on the patellofemoral joint mechanics of young women during stair descent, the Vicon infrared three-dimensional motion capture system and a three-dimensional force platform were used to synchronously collect kinematic and kinetic data of the knee joints from 25 young women descending stairs while wearing high-heeled shoes with four different heights (1, 3, 5 and 7 cm). Digital formulas were adopted to calculate indexes such as the patellofemoral joint stress and patellofemoral joint reaction force of young women. The data were statistically analyzed by using one-way repeated measures ANOVA. The results showed that compared with the flat heel condition, significant increase was observed in the second peak of patellofemoral joint stress, patellofemoral joint reaction force, quadriceps force and knee extension moment in the high heel, medium heel, and low heel conditions. Therefore, when women descend stairs, wearing high-heeled shoes with a heel height of less than 3 cm can effectively reduce the patellofemoral joint reaction force and patellofemoral joint stress, which may help to decrease the likelihood of patellofemoral joint pain.
